How to access it (65 and over)

Contact My Aged Care
Call My Aged Care on 1800 200 422 or apply online. They will ask some questions about how you are managing at home.

Have an assessment
A free assessment (usually in your home) works out what help you need and which services you are eligible for.

Receive your funding level
You are notified of your approved services and funding. A contribution may apply based on a Services Australia income and assets assessment.

Choose My Centred Care
Send us your referral or assessment details. We meet you, agree a plan and prices in writing, and start support.

Personal care
Showering, dressing, grooming, continence support and mobility assistance.
Clinical & nursing care
Registered and Enrolled Nurse visits, wound and medication support, care plan reviews.
Care management
A named care manager coordinating your services, reviews and paperwork.
Social support & community engagement
Companionship, outings, groups, and support to stay connected to what you enjoy.
Shopping & appointments
Shopping assistance and accompanied appointments, with transport where needed.
Home maintenance & light gardening
Low-risk upkeep that keeps the house and yard safe and manageable.
Respite support
In-home and community respite so carers and family can take a break.
