My Centred Care is an Adelaide-based provider delivering both NDIS supports and Support at Home aged care services. We were built on a simple idea: the person receiving support should be the one shaping it.

That means we take the time to understand your routine, your household and what "a good day" looks like for you — then we match you with workers who fit, and keep the same faces coming back. Our care managers stay close to every plan, and registered nurses provide clinical oversight where it is needed.

What is the NDIS?

The National Disability Insurance Scheme (NDIS) is the Australian Government scheme that funds reasonable and necessary supports for people living with a permanent and significant disability. Funding is attached to you, not to a provider — so you choose who delivers your supports and how.

Who can we support?

  • Participants with an approved NDIS plan, aged 7 to 65
  • Plans that are self-managed, plan-managed or NDIA-managed
  • Participants living at home, in shared living or with family
  • Participants who want to change providers and keep continuity of care

    Personal care, showering and dressing, medication prompting, morning and evening routines.

    Cleaning, laundry, linen, meal preparation and general help keeping the home running.

    Support to travel to appointments and activities, including transport in a staff vehicle.

What is Support at Home?

Support at Home is the Australian Government aged care program that funds services in your own home — replacing the old Home Care Packages and CHSP arrangements. After an assessment, you are given a level of funding and you choose an approved provider like My Centred Care to deliver the services in your plan.

How to access it (65 and over)

    Contact My Aged Care

    Call My Aged Care on 1800 200 422 or apply online. They will ask some questions about how you are managing at home.

    Have an assessment

    A free assessment (usually in your home) works out what help you need and which services you are eligible for.

  3. 3

    Receive your funding level

    You are notified of your approved services and funding. A contribution may apply based on a Services Australia income and assets assessment.

    Choose My Centred Care

    Send us your referral or assessment details. We meet you, agree a plan and prices in writing, and start support.

These are the prices My Centred Care most frequently charges for Support at Home services. Prices include direct worker time, rostering, administration, supervision, compliance, insurance, quality management, standard provider travel within the Adelaide metropolitan area and a reasonable operating margin.

Terms and conditions

Agreement before services

Prices are discussed and agreed with each participant or representative before services commence, and recorded in the Service Agreement or written confirmation.

No separate routine administration or provider travel fee

Routine administration and standard provider travel within the Adelaide metropolitan area are included in the listed service price and are not charged separately.

Minimum booking and billing increments

The standard minimum booking is 1 hour. After the minimum booking, services are generally billed in 15-minute increments unless otherwise agreed. Longer minimum bookings may apply for complex supports, weekends, public holidays or long-distance travel.

Cancellations and no-shows

We ask for at least 24 hours’ notice to cancel or change a service. With less notice, or if the participant is not available at the agreed time and place, MCC may charge up to 100% of the scheduled fee where the worker cannot reasonably be redeployed. Nursing, group activities, transport and subcontracted services may require 48 hours’ notice. Fees generally do not apply for circumstances outside the participant’s reasonable control, such as unplanned hospitalisation or medical emergency.

Participant contributions

You may be asked to pay a contribution depending on your Support at Home contribution assessment, the service type and how much service you receive. Rates are determined by an income and assets assessment through Services Australia, or the Department of Veterans’ Affairs where applicable. Participants generally do not contribute to clinical supports such as nursing care.

Out-of-pocket expenses, goods and equipment

Parking, tolls, entry fees, tickets, meals, shopping items and requested cleaning products are not included unless stated, and are agreed before purchase. Household items, groceries, continence products, clinical consumables, equipment, assistive technology and home modifications are not included in hourly rates unless specifically agreed.

Self-managed participants and third-party providers

Where a self-managed participant chooses their own third-party provider or worker and MCC agrees to arrange or manage that service, an administration/overhead fee of up to 10% of the third-party price may apply. Any third-party price and overhead is agreed in writing before the service is confirmed.

Subcontracted or externally sourced services

Where MCC arranges an external provider or contractor to deliver an approved service on our behalf, this is discussed and agreed with you beforehand. MCC remains responsible for oversight of services delivered on its behalf.

Complex or high-risk services

MCC may require a risk assessment, care plan review, additional staff, RN oversight, specialist equipment or a quoted price before confirming complex or high-risk services.

Price review

MCC reviews published prices regularly so they stay consistent with Support at Home requirements, operating costs and applicable legislation. Where prices change, participants or representatives are notified in advance where required.

GST

Prices are GST inclusive where GST applies. Government-funded aged care services may be GST-free depending on the service and funding arrangement.

Quality and safety

All services are delivered in line with the Aged Care Code of Conduct, applicable Aged Care Quality Standards, MCC policies and procedures, staff scope of practice, worker screening requirements and clinical governance requirements where relevant.

Service notes

  • Support workers do not perform clinical nursing tasks unless appropriately qualified, trained and authorised under MCC clinical governance processes.
  • Registered Nurse services are delivered by an AHPRA-registered Registered Nurse within scope of practice.
  • Enrolled Nurse services are delivered by an AHPRA-registered Enrolled Nurse under Registered Nurse supervision, where clinically appropriate and within scope of practice.
  • Home maintenance and light gardening cover low-risk tasks only. Building, electrical, plumbing, roofing, tree lopping, pest control and other licensed or high-risk work is excluded unless arranged through an appropriately qualified provider and agreed before service.
  • Participant transport is different from provider travel. Provider travel to attend routine metro services is included in the hourly rate; participant transport applies when the participant travels in an MCC or staff vehicle.
  • Group social support may include outings, cafe visits, walking groups, craft, games, shopping groups and other agreed group activities. Activity costs, meals, entry fees, parking and transport are not included unless stated, and are agreed before the activity.

    We meet you (at home if you prefer), agree the supports, workers and prices, and put it in writing.

    Services begin with a named care manager who stays in touch and reviews the plan as things change.
